Patient Encounter and Documentation

After the patient encounter, the provider documents the conditions observed and treatments provided. This clinical documentation serves as the foundation for all subsequent billing activities.

Coding and Claim Creation

Medical coders then translate this documentation into standardized diagnosis codes (ICD-10) and procedure codes (CPT), which communicate to payers exactly what was diagnosed and what services were rendered. The medical billing specialist enters all this information into the practice management software, creating a claim that can be submitted electronically to the insurance payer or clearinghouse.
